You have a pathological connection between the pulmonary arteries and the pulmonary veins.
The heart pumps oxygen-poor blood through the pulmonary arteries into the body. Inside the lungs, the blood flows through very small blood vessels. These small blood vessels have a thin, transparent wall. Through this wall, oxygen can be absorbed into the blood. The oxygen-poor blood then flows back to the heart through the pulmonary veins.
Due to a pathological connection between pulmonary arteries and pulmonary veins, the blood flows past the small blood vessels. No oxygen can be absorbed into the blood as a result. These connections between the pulmonary arteries and the pulmonary veins are usually congenital.
